Focus On Health A Physical Therapy Corporation
Notice of Privacy Practices
California-Compliant•Original Effective Date: July 16, 2026•Revised Effective Date: October 8, 2026
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Focus On Health A Physical Therapy Corporation (the “Practice,” “we,” or “us”) is required by law to maintain the privacy of your protected health information (PHI), to provide you this Notice of our legal duties and privacy practices, to notify you following a breach of unsecured PHI, and to abide by the terms of the Notice currently in effect. Where the California Confidentiality of Medical Information Act (Civil Code § 56 et seq.) or other state or federal law is more protective of your information than HIPAA, we follow the more protective law.
How We May Use and Disclose Your Health Information Without a Separate Authorization
Treatment
We may use and disclose your PHI, for example, to your referring physician or another health care provider involved in your care to coordinate your physical therapy care.
Payment
We are a private-pay practice and do not bill commercial insurance. We may use and disclose your PHI as necessary for payment-related activities associated with the health care services we provide. This may include processing payment for services provided, providing a receipt showing amounts charged and paid upon request, maintaining payment records, and collecting amounts owed.
If an account becomes delinquent and remains unpaid despite reasonable collection efforts, we may disclose limited billing information (the minimum necessary protected health information), as permitted by applicable federal and California law, to collection agencies, our business associates, or courts to collect amounts owed. Consistent with applicable federal and California law, we will not furnish PHI or medical-debt information to any consumer credit reporting agency.
Health Care Operations
We use your PHI for quality assessment, staff review, licensing, business management, and to contact you about appointments, scheduling, and payment at the phone number(s) and email you provide.
Appointment Reminders & Communications
We may contact you with appointment reminders, scheduling, and other brief administrative messages by phone, voicemail, text, or email according to the preferences you give us. You may request confidential communications by alternative means or at alternative locations, and we will accommodate reasonable requests. Appointment reminders, scheduling, and payment communications are part of our health-care operations and do not require your separate authorization. We do not receive payment from third parties to send you marketing communications.
Business Associates
We may share PHI with vendors who perform services for us (e.g., recordkeeping, IT, billing/receipt software) under written contracts requiring them to safeguard your information.
Persons Involved in Your Care
With your agreement, or using professional judgment in an emergency, we may share limited PHI with family, friends, or others you involve in your care or payment.
Uses and Disclosures Permitted or Required by Law
We may use or disclose PHI without your authorization when required or permitted by law, including: as required by law; public health activities; reporting suspected abuse, neglect, or domestic violence; health oversight activities; judicial and administrative proceedings (subpoena/court order); law enforcement purposes; coroners, medical examiners, and funeral directors; organ/tissue donation; research under approved protocols; to avert a serious threat to health or safety; disaster relief purposes; specialized government functions (e.g., military, national security); and workers’ compensation as authorized by law.
Uses and Disclosures That Require Your Written Authorization
Most uses and disclosures of psychotherapy notes, uses of PHI for marketing, and any sale of PHI require your written authorization. Other uses not described in this Notice will be made only with your written authorization, which you may revoke in writing at any time (except to the extent we have already relied on it). California law provides heightened protection for mental-health, HIV/AIDS, substance-use-disorder, and reproductive/gender-affirming care information, and we will not disclose those categories except as specifically authorized or as required by law.
To the extent that we have substance use disorder patient records subject to 42 C.F.R. Part 2, we will not use or disclose information in those records in civil, criminal, administrative, or legislative investigations or proceedings against you without your written consent or a court order and subpoena, as permitted by law.
Your Rights Regarding Your Health Information
Access & Copies
You may inspect and obtain a copy of your records. Under California law we allow inspection within 5 business days and provide copies within 15 days of a written request. We may charge a reasonable, cost-based fee for copying and providing records, as permitted by applicable federal and California law.
Amendment
You may request in writing that we amend PHI you believe is incorrect or incomplete; we may deny the request in limited circumstances and will explain any denial.
Accounting of Disclosures
You may request a list of certain disclosures we have made (excluding those for treatment, payment, operations, and those you authorized). The first accounting requested in any 12-month period is provided free of charge; additional requests may be subject to a reasonable, cost-based fee.
Restrictions
You may request restrictions on certain uses and disclosures. Because we do not bill insurers, your records are generally not disclosed to health plans; we will honor a request to restrict disclosure to a health plan for services you pay for in full out of pocket.
Confidential Communications
You may request that we communicate with you by alternative means or at an alternative location.
Paper Copy
You may obtain a paper copy of this Notice at any time, even if you agreed to receive it electronically.
Breach Notification
You will be notified if a breach of your unsecured PHI occurs, as required by HIPAA and California law (Civil Code §§ 1798.29, 1798.82). If a breach occurs, we will notify you without unreasonable delay and no later than 60 days after discovery, or as otherwise required by law; the notice will describe the breach, the information involved, the steps you should take, and what we are doing in response.
Changes to This Notice
We may change this Notice and make the revised Notice effective for all PHI we maintain. The current Notice will be posted in our office and at https://focusonhealthpt.com/.

If you believe your privacy rights have been violated, you may file a written complaint with our Privacy Officer or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
