Good Faith Estimate Notice

If you are uninsured or are paying for your health care directly, you have the right to receive a Good Faith Estimate of the expected charges for scheduled or requested items or services. You may request a Good Faith Estimate before scheduling care or when scheduling services in advance. If a bill is at least $400 more than the amount shown on your Good Faith Estimate, you may have the right to use the federal patient-provider dispute resolution process.
